We hope this website will be a useful resource for teachers, high school staff, students, their families and service agencies helping high school students prepare for adult life after high school.
What is Transition Planning?
For students with disabilities and their families, this change can be particularly challenging, overwhelming and exciting.
To experience transitions to satisfying adult lives, there must be teamwork among students, parents, educators and others.
Depending on the student’s needs, what follows high school may be college, a job, or some other alternative. Without a sound transition plan in place, a disabled student is at risk for falling through the cracks and becoming an individual who does not go on to be as successful as the general population.
What Is Covered In A Transition Plan?
Employment & Career Interests
Along with assisting the student towards sound career and occupational paths, the transition team may also need to look into the need for adult employment and workshop services.
Post-Secondary Education
There will be a variety of options to consider including: university, community college, adult education or vocational training programs. High school and post secondary students with disabilities need to be aware of their rights and the services available to them. It is important that transition planning teams take into account all aspects of a student's educational needs.

What Does the TPC Do?
The Transition Planning Committee:
- Identifies current transition services
-Facilitates development of IEP interagency plan teams
-Provides for exchange of information
-Develops information and special events for student, family and school
-Files annual report
