INTAKE INFORMATION
The following information covers the application process for services at Fulton County
Rehabilitation Center, INC. (FCRC) including other agencies that may have an effect on the process. This material is intended to give a brief overview for interested individuals, their families, and/or support agencies.
If a person is interested in seeking our services, they should contact Bill Whitehouse at
647-5555, between 8:00 A.M. to 3:30 P.M., Monday through Friday to schedule an appointment.

PROGRAM DESCRIPTIONS
All programs are licensed and funded
by the Department of Human Services to provide the following services:
A. Developmental Training (DT) Program: The Developmental Training Program serves individuals that are functioning in the lower moderate to severe and profound range of mental impairment. Enrollment in this program requires a waiver or certain residential placements for a person to receive services. Eligibility for Public Aid, Page 5, is a requirement for the waiver and many residential placements. WISC. Page 5, is responsible for the waiver process.
The Developmental Training Program provides active treatment with training in motor development, personal care, language and vocational training. See the Developmental Training Program Brochure for additional information.
B. Regular Work Program (RW): The Regular Work Program serves individuals that are functioning at mild or upper moderate range of mental impairment. Enrollment in this program currently does not require a waiver. The Regular Work Program provides employment and training in work and social skills with the goal of the individual being able to reach his or her highest level of independence. See the Regular Work Program Brochure for additional information.
C. Supported Employment Program: The Supported Employment Program provides support to those individuals seeking work in the community. A job coach may be used to smooth the transition from sheltered to community employment. Individuals usually apply for this program from the RW program.
