What is Transition Planning?

Transition planning is looking ahead and preparing for what happens after high school.

For all young people, the transition from student life in school to adult life in the community is complicated.

For students with disabilities and their families, this change can be particularly challenging, overwhelming, and exciting. For young people to experience transitions to satisfying adult lives there must be teamwork among students, parents, educators and others.

Depending on the student’s needs, what follows high school may be college, a job, or some other alternative. Without a sound transition plan in place, a disabled student is at risk for falling through the cracks and becoming an individual who does not go on to be as successful as the general population.
